Please also complete a Quality of Life assessment.  We encourage you to complete both assessments in advance of your consultation. Doing so will help you get the most out of your time with our Post-COVID specialists.

These assessments allows our specialized care team to better understand the details of your specific case. We look forward to helping you move forward on your Post-COVID recovery journey.


Please ensure to complete these steps before your Initial Consultation. We have broken these up into two shorter Assessments to make it easier for you to complete. At any time please feel free to take breaks and pace yourself.

Step B

Complete Quality of Life Assessment


This assessment gives our medical team a more indepth look at your indiviudal needs.


We use Microsoft Teams for our Welcome Calls. We recommend downloading as well as testing your ability to Login and use Microsoft Teams at least 5 minutes before your scheduled call time. What if I did not receive any confirmation email?

What is your cancellation and refund policy?

Cancellations made before 72 hours of the appointment are eligible for a full refund.

Cancellations made before 48 hours of the appointment are eligible for a 50% refund.

Cancellations made within 24 hours of the appointment are non-refundable.

If the intake questionnaire is not submitted 12 hours before your appointment, we will not have the information required to complete the appointment and there will be a $50 rebooking charge.

Do you provide Emergency Treatment? Are there symptoms you do not treat?

This is a virtual clinic and is not intended for urgent or emergency care. Seek emergency medical attention if you experience new onset chest pain especially if it feels like heaviness or pressure, a rapid heart rate, blueish or grey lips, skin, or nail beds, new onset or worsening confusion especially if it’s paired with drowsiness or new headache, moderate to severe abdominal pain, new onset trouble breathing or shallow, rapid breathing.
